Description

Health equity means everyone has a fair shot at living the healthiest life possible, and one of the first steps in ensuring that everyone has a fair shot is working with community partners to identify and address health disparities. A community-driven health equity action plan is intended to create a roadmap for communities to take action on a health equity agenda that addresses priorities in their community. This training will examine how communities can identify and prioritize health disparities and address the social determinants of health, facilitate a community change process through authentic community engagement, and collect and analyze data on health disparities to identify key issues to inform a health equity action plan.
